By creating an account, purchasing a test, or using any part of the screenings4u marketplace, you confirm that you have read, understood, and agree to these Terms and our Privacy Policy. If you do not agree, you must immediately cease all use of our services and this website.
2. Description of Services
screenings4u provides a digital marketplace connecting users with third-party laboratory collection sites. Our services include facilitating the purchase of drug and alcohol screenings, generating digital donor passes, providing secure portal access for results, and offering DOT compliance management tools. screenings4u performs collections at our locations in Chicago, IL.
3. Eligibility & User Accounts
You must be at least 18 years old to use this platform. Users are responsible for maintaining the confidentiality of their account credentials. screenings4u reserves the right to suspend or terminate accounts that provide false information or violate security protocols.
4. Scheduling & Cancellations
Tests must be scheduled through our online system. Digital donor passes are valid for a specific period as noted at the time of purchase. Cancellations and refunds are subject to our refund policy, which generally prohibits refunds once a donor pass has been generated or a test has been initiated at a collection site.
5. Pricing & Payments
All prices are listed in USD. Payment is required in full at the time of checkout. We use secure third-party payment processors; screenings4u does not store full credit card details. Prices are subject to change based on lab network fees and compliance requirements.
6. Testing Process & Limitations
Donors must present their digital donor pass and a valid government-issued ID at the collection site. Testing accuracy is dependent on laboratory protocols and donor cooperation. screenings4u is not liable for test results that are inconclusive due to 'shy bladder,' specimen tampering, or improper collection techniques at third-party sites.
7. Results & Confidentiality
Results are delivered through our secure, encrypted digital portal. We adhere to privacy standards to ensure your health information is protected. By using our service, you authorize screenings4u to receive and transmit your results to the designated employer or individual account holder.
8. Employer Responsibilities
Employers using screenings4u for workplace compliance are responsible for ensuring that their testing policies comply with local, state, and federal labor laws, including the Fair Credit Reporting Act (FCRA) where applicable.
9. DOT & Legal Compliance
For Department of Transportation (DOT) regulated testing, screenings4u facilitates protocols that meet FMCSA, FAA, FRA, FTA, PHMSA, and USCG standards. However, the final responsibility for regulatory compliance lies with the designated DER (Designated Employer Representative).
10. Limitation of Liability
To the maximum extent permitted by law, screenings4u shall not be liable for any indirect, incidental, or consequential damages resulting from the use of our platform or testing results. Our total liability is limited to the amount paid for the specific service in question.
11. Data & Privacy

12. Governing Law
These Terms are governed by and construed in accordance with the laws of the State of Illinois, USA. Any disputes shall be resolved in the courts located in Chicago, Illinois.
