What Comes with Our DOT SAP Evaluation: Detailed assessment and evaluation to determine drug and alcohol use history and treatment needs. Creation of a personalized treatment plan based on your evaluation. Detailed follow-up assessment and evaluation. Creation of a personalized follow-up plan based on your compliance with the treatment plan. Assistance with administrative requirements and documentation for reinstatement. Ongoing support and resources for recovery and compliance with DOT regulations. Creation of a follow-up testing plan after you return to duty. Payment: The total cost of our DOT SAP Evaluation is $595. This payment is split into two payments of $297.50. The first payment of $297.50 is due when you sign up, and the second payment of $297.50 is due on or before you attend the first SAP evaluation. Attend the SAP evaluation: This SAP evaluation is online; you will receive a link to join a one-on-one meeting for the date you schedule.
