This document outlines the terms, conditions, and notices (the "Terms") applicable to your use of our Site. By accessing or using www.screenings4u.com, you agree to be bound by these Terms. SAMHSA-certified laboratories perform all laboratory services, and screenings4u is fully licensed and/or certified for all services provided.

Electronic Communications

By communicating with us through the website or via email, you consent to receive electronic communications. These communications satisfy any legal requirements for written communications.

 

User Account Responsibilities

 

You are responsible for maintaining the confidentiality of your account credentials and limiting access to your computer.

You must accept responsibility for all activities conducted under your account.

Account transfers to third parties are not permitted.

The Company is not liable for unauthorized access due to account theft or misappropriation.

 

Age Restrictions

We do not knowingly collect information from anyone under the age of thirteen. Users under 18 should only access the website with parental or guardian approval.

 

Cancellation and Refund Policy

  • A non-refundable registration/donor pass is required for all our services, including drug, alcohol, DNA, and background tests.

  • While tests cannot be refunded, they may be rescheduled or credited for future use.

 

DOT Random Drug and Alcohol Consortium

  • Upon receipt of payment, a certificate of compliance will be emailed.

  • DOT drug tests may require a Federal paper custody and control form, which will be mailed to the recipient.

  • Service availability may vary based on location and state regulations, and customer service will assist in providing alternative lab testing options if needed.

 

Instant Drug Testing Information

Instant drug tests yielding negative results are available on the same day, while non-negative results may be sent to one of our SAMHSA-certified laboratories for confirmation testing and MRO review with your consent. There will be a $20.00 fee to send all Instant Tests to the lab for testing.

Billing Procedures

1. All invoices for onsite testing services are due within thirty (30) days of receipt. All invoices for post-accident are due before we come out and before we perform any services.

 

2. FormFox Marketplace

All services coming from the FormFox Marketplace are due within thirty (30) days.

 

Background Check and Drug Test Result Disputes

 

Understanding the Report

Before initiating a dispute, carefully review the results of your background check or drug test. Ensure that you understand the information presented and compare it with your personal records.

 

Documenting Concerns

If you notice any discrepancies or have concerns about the accuracy of the information, document your findings meticulously. This could include noting specific errors, gathering relevant personal documentation, and preparing any necessary evidence to support your case.

 

Initiating the Dispute

Contact screenings4u via customer support channels. This can typically be done via email or phone. Clearly state your intention to dispute the results and provide detailed information about the inaccuracies you’ve identified. It’s beneficial to reference the report number, specimen ID, and test date.

 

Providing Evidence

Along with your dispute request, submit any evidence you have collected that supports your claim. This might include identification documents, court records, or any other relevant information that verifies your statements.

 

Review Process

Once your dispute has been submitted, screenings4u is expected to respond promptly. We will review your case, which might involve reaching out to third-party sources, conducting further investigations, or verifying the original data provider’s information.

 

Receiving Results

After completing our review, screenings4u will communicate its findings back to you. If we agree with your dispute, we will issue a corrected report. If we do not find sufficient evidence to support your claim, we will provide you with a clear explanation of our decision.

 

Further Actions

If you are unsatisfied with the resolution from screenings4u, you may have additional options. Consider consulting legal advice, especially if the reported information is affecting your employment or other critical aspects of your life. You can also report serious discrepancies to relevant regulatory bodies or consumer protection agencies.

 

Keeping Records

Throughout the dispute process, maintain thorough records of all communications, documents submitted, and responses received. This can be crucial for any follow-up actions or future disputes.

Third-Party Links and Services

  • Our Site may contain links to third-party sites. We do not have control over these sites and are not responsible for their content.

  • Third-party organizations may deliver services provided through our site, and you consent to share necessary data with them.

 

Intellectual Property Rights

  • You are granted a non-exclusive, revocable license to use the Site.

  • You agree not to modify, distribute, or exploit any content from the Site without written permission from screenings4u.

 

International Users

If accessing the Site from outside the US, you are responsible for complying with local laws.
