Click on the Personal Phone field and type in the user’s phone number

Click on the User’s Role(s) field and select a role for the user. In this case, we will select “Hiring Manager (Ordering Only)” because we only want this person to order tests.

Click on the Location field, if you have multiple locations, they will populate here. You will select the location where you want the Hiring Manager to order tests.

Look at the "Should we notify you about services and events?" section and select the No toggle to turn on notifications.

Click the No toggle button to turn on each notification you want a notification for. In this case, we will turn on notifications for NON-REGULATED DRUG TESTING (Urine).

Step 30: Click the "Print PDF" button; this is the authorization form your employee will need when they visit the collection site.

Step 31: Click the input field to enter the employee's email address. An authorization form will be sent to the employee's email address, which they can present upon arrival with their ID. Alternatively, they can show the barcode sent in the same email to the collector and provide their ID.
